Types of Palatal Expanders (RPE, MARPE & MSE) Explained

Hearing that you or your child may need jaw expansion can feel overwhelming, especially when you’re trying to understand the difference between treatment options.

At Senties Family Orthodontics in San Antonio, TX, Dr. Senties helps patients of all ages navigate these decisions with clarity and confidence.

We offer several types of palatal expanders designed for different ages and stages of development, and the benefits of treatment have been life changing for many patients.

Let’s dive into the RPE (Rapid Palatal Expander), MARPE (Miniscrew-Assisted Rapid Palatal Expansion), and MSE (Maxillary Skeletal Expansion) appliances:

What is a palatal expander?

A palatal expander is a specialized orthodontic appliance designed to gently widen the upper jaw.

By creating space at the mid-palatal suture (where the two halves of the upper jaw meet), expansion can:

  • Make room for crowded teeth
  • Improve bite alignment
  • Support better nasal breathing and airway function

The key difference between palatal expander types comes down to how they anchor and whether the jaw is still growing.

Exploring the RPE: Rapid Palatal Expander (For Children)

Creating Space While Kids are Still Growing

Who the RPE is for:

How the RPE works:

  • The rapid palatal expander (RPE) gently widens the upper jaw while it’s still developing. The appliance is attached to the upper molars, and a small screw is turned periodically to create gradual expansion over time.

Benefits of the RPE

  • Expanding the upper jaw early can create room for crowded teeth, improve bite alignment, and help guide healthy jaw development.

Limitations of the RPE

  • Because this approach depends on growth, it works best before the upper jaw bones fuse together. Older teens and adults typically require different expansion methods, such as MARPE or MSE.

Exploring MARPE: Miniscrew-Assisted Rapid Palatal Expansion (For Older Teens & Adults)

Skeletal Expansion Without Traditional Surgery

Who MARPE is for:

  • Older teens and adults whose jaws have finished most of their growth.

How MARPE works:

  • MARPE uses a specialized expander supported by small titanium miniscrews placed in the palate. By anchoring directly to the bone, the appliance can create expansion that affects the jaw itself, rather than simply moving the teeth outward.

Benefits of MARPE:

  • MARPE can create additional space for crowded teeth, improve bite alignment, and achieve true skeletal expansion in patients who are no longer candidates for a traditional RPE. In some cases, it may also help improve airflow through the nasal passages.

MARPE is typically best for:

  • Older teens and adults with narrow upper jaws who need expansion but may be able to avoid more invasive surgical procedures.

A Specific Type of MARPE

MSE (Maxillary Skeletal Expander) is a specific model of MARPE. Like other MARPE appliances, it uses miniscrews placed in the palate to achieve skeletal expansion of the upper jaw in older teens and adults.

During your consultation, Dr. Senties will determine whether MSE or another MARPE design is the most appropriate option based on your unique anatomy and treatment goals.

Signs You or Your Child Might Benefit from a Palatal Expander

Not sure if palatal expansion is necessary?

While only an orthodontic evaluation can determine whether expansion is appropriate, there are several signs that may indicate the upper jaw isn’t developing wide enough to comfortably accommodate the teeth and bite.

Here are some common things we look for at different ages:

Children (Potential RPE Candidates)

Children who may benefit from a rapid palatal expander (RPE) often show signs such as:

  • Crowded baby teeth or permanent teeth that don’t seem to have enough room to erupt properly
  • A crossbite, where some upper teeth fit inside the lower teeth when biting down
  • Frequent mouth breathing or snoring during sleep
  • A narrow smile or a high, arched palate
  • A history of thumb-sucking or prolonged pacifier use that has affected jaw development

Teens (Potential MARPE Candidates)

As growth begins to slow, signs of a narrow upper jaw may include:

  • Persistent crowding, even after all baby teeth have been lost
  • A crossbite or uneven bite that affects comfort or chewing
  • Difficulty breathing through the nose or chronic mouth breathing
  • Teeth that overlap, rotate, or continue shifting because there isn’t enough space in the dental arch

Adults (Potential MARPE or MSE Candidates)

Adults with a narrow upper jaw often experience concerns that go beyond crowded teeth, including:

  • Long-standing crowding or bite issues that clear aligners or braces alone may not fully address
  • A narrow upper arch associated with snoring, airway concerns, or sleep-disordered breathing
  • An uneven bite that affects comfort or function
  • Being told that jaw surgery may be needed and wanting to explore non-surgical or minimally invasive alternatives first

If any of these signs sound familiar, Dr. Senties can evaluate your smile, bite, and jaw development to determine whether a rapid palatal expander (RPE), miniscrew-assisted expansion (MARPE), or a maxillary skeletal expander (MSE) may be the right fit.

Which type of palatal expander is right for you?

Ultimately, the answer depends on a few key factors:

  • Your age and growth stage
  • Whether the palatal suture has fused
  • Severity of the jaw width discrepancy
  • Your goals for function, health, and aesthetics

At your consultation, Dr. Senties will perform a comprehensive exam with 3D imaging and airway evaluation to determine the best path forward.
